Yalent is seeking a dedicated Psychiatric Nurse Practitioner (NP) to join our client, AllCare , in delivering compassionate, patient-centered mental healthcare to seniors. In this pivotal role, you’ll provide evidence-based psychiatric care through AllCare’s tech-enabled platform, crafting personalized treatment plans, managing medication regimens, and fostering therapeutic relationships—all while minimizing administrative tasks. Collaborating with a multidisciplinary team, you’ll drive mental health outcomes in a coordinated care model, upholding AllCare’s commitment to innovation and quality in senior behavioral health.

Key Responsibilities:

• Provide psychiatric evaluations, diagnoses, and treatment for senior patients in Assisted Living Facilities.
• Develop and implement individualized care plans, integrating medical, psychosocial, and behavioral health strategies.
• Collaborate with AllCare’s physicians, primary care providers, and medical concierge team to ensure comprehensive and coordinated care.
• Utilize our technology platform to track outcomes, streamline workflows, and enhance care quality.
• Participate in quality improvement initiatives focused on reducing hospitalizations, improving mental wellness, and elevating the patient experience.
• Maintain accurate and timely documentation in the electronic health record system.

Requirements

• Active California licensure as a Nurse Practitioner (NP) with Psychiatric-Mental Health Nurse Practitioner (PMHNP) certification.
• 2+ years of clinical experience in Psychiatry, Geriatrics, or Primary Care (geriatric psychiatry strongly preferred).
• Strong commitment to compassionate, patient-centered care for elderly populations.
• Proficiency with digital health tools, telemedicine, and electronic health records (EHR).
• Collaborative mindset and excellent communication skills for multidisciplinary care coordination.
